This unusual item measures 2 1/4" - 3" X 6 1/4" (57mm - 76mm X 159mm) and is an in-line strain insulator designed to hold a fuse - the top two holes were used for bolts with a fuse between them and the first wire going through the bottom hole and the second around the side.  Picture courtesy Don Mecklenborg.  Below is a picture if this style insulator with hardware attached courtesy of Brian Rieker.
