This small item can be mistaken for an insulator but it is designed as a cable guide for window weights. It measures about 2" long and 1" wide. It was patented as US Patent 83,685 by Alfred Bicknell of South Reading, Mass as an alternative to the pulleys previously used. Here is the drawing from the patent:
